This slide deck is a communication tool that can be used one-on-one with an individual patient or in a group patient setting to translate the complexities of the latest evidence and information on prostate cancer staging and treatment.

  Patient Evaluation Survey Patient Online Evaluation

Evaluation Surveys: Separate surveys for patients and presenters to evaluate whether this program provided informative, relevant, and timely information for the patient. The surveys can be completed online or sent as a PDF.
PCE Oncology will donate $5 to the Prostate Cancer Foundation for each survey returned (up to a total of $4000).
